CenterWell Senior Primary Care Bringing Personalized Care to Virginia Seniors

CenterWell Senior Primary Care® will open eight new senior-focused primary care centers in Virginia in 2023 and 2024. This will mark the company’s debut in Virginia, giving local seniors access to CenterWell’s personalized, care-team approach to healthcare. The first center grand opening is Oct. 25 at CenterWell Chesapeake from 10 a.m. to 1 p.m.

The upcoming grand opening at CenterWell Chesapeake will include tours of the center, music, games, and free light fare from Hot Mama’s Kitchen food truck.

CenterWell Senior Primary Care is one of the largest, fastest-growing senior-focused, value-based care providers in the country. Together with its sister brand Conviva Care Center, the businesses make up Humana’s Primary Care Organization, delivering care to more than 270,000 seniors in nearly 260 centers as of June 30, 2023. Centers are now open in 15 states with plans to add 30-50 new centers per year through 2025.

CenterWell Senior Primary Care locations in Virginia are staffed by board-certified physicians, nurse practitioners and medical assistants, and patients will have easy access to care team coordinators, behavioral health specialists, social workers, a clinical pharmacist and referral specialists. The entire team has been specially trained to treat the senior population. In addition to its healthcare offerings, CenterWell facilities include activity centers for local seniors.

CenterWell Chesapeake, located at 1501 Sentinel Dr., Suite 407, began accepting patients in September.

In Richmond, grand opening celebrations will be Nov. 29, for CenterWell Midlothian, located at 6900 Midlothian Turnpike in Richmond, and Nov. 30, for CenterWell South Laburnum, located at 4722 S. Laburnum Avenue in Henrico.

Other centers with planned openings in Virginia in the next several months include a third location in Richmond, CenterWell Brook Road; and CenterWell Hampton, CenterWell Tidewater, CenterWell Portsmouth and CenterWell Southeast Newport News in the Tidewater area of Virginia.
