Hands of Hope, Agape Care Group’s multi-state home-based pediatric program, has achieved program certification from CHAP, Community Health Accreditation Partner. One of only nine hospice programs in the nation to achieve distinctive pediatric certification, Hands of Hope offers palliative and concurrent hospice care for children ages 0-21 in South Carolina, Georgia, and in several areas of North Carolina. The Hands of Hope team works closely with the child’s existing care teams to develop personalized treatment plans for life-limiting illness. They provide specialized support for the physical, emotional, and spiritual needs of every patient and their family.

CHAP is an independent, nonprofit accrediting body for home and community-based healthcare organizations. The CHAP Board of Review issued certification to Hands of Hope based upon on-site visit, evaluation of program practices against nationally recognized Pediatric Certification standards, and submission of an acceptable Development Plan.
